| 目的：探讨养阴清肺活血方对气阴两虚型非小细胞肺癌（NSCLC）放疗后放射性肺损伤的临床效果，及其对血清炎性因子水平、转化生长因子－β1（TGF－β1）及肿瘤标志物的影响。方法：选取符合纳入标准的气阴两虚型NSCLC患者286例，依据随机数字表随机分为观察组和对照组，每组各143例。对照组给予常规调强适形放射治疗，观察组在对照组治疗基础上口服养阴清肺活血方，两组均治疗12周。比较两组患者临床疗效及治疗前后中医症状积分、血清炎性因子、TGF－β1和肿瘤标志物的变化情况。结果：观察组客观缓解率72.03%（103/143）及疾病控制率93.71%（134/143）显著高于对照组客观缓解率55.94%（80/143）及疾病控制率83.92%（120/143）（P < 0.05）。治疗后，两组中医各症状积分和血清非小细胞肺瘤相关抗原21－1（CYFRA21－1）、癌胚抗原（CEA）水平均显著降低（P < 0.05），IL－6、TNF－α及TGF－β1水平显著升高（P < 0.05），且观察组中医各症状积分和CYFRA21－1、CEA、IL－6、TNF－α及TGF－β1水平显著低于对照组（P < 0.05）。观察组不良反应发生率14.69%（21/143），显著低于对照组28.67%（41/143）（P < 0.05）。结论：养阴清肺活血方治疗气阴两虚型NSCLC放疗后放射性肺损伤临床效果显著，能够改善中医临床症状，降低炎性因子及肿瘤标志物水平，并减轻不良反应。 |

| Objective： To investigate the clinical effects of Yangyin Qingfei Huoxue Formula on radiation-induced lung injury in non－small cell lung cancer （NSCLC） patients with Qi－Yin deficiency syndrome after radiotherapy， and its effects on serum inflammatory factor levels， transforming growth factor－β1 （TGF－β1）， and tumor markers.Methods： A total of 286 NSCLC patients with Qi－Yin deficiency syndrome who met the inclusion criteria were selected and randomly divided into an observation group and a control group， each consisting of 143 patients. The control group received conventional intensity－modulated radiotherapy， while the observation group was treated with Yangyin Qingfei Huoxue Formula in addition to the treatment of the control group for 12 weeks. The clinical efficacy and changes in TCM symptom scores， serum inflammatory factors， TGF－β1， and tumor markers were compared between the two groups before and after treatment.Results： The observation group had a significantly higher objective response rate of 72.03% （103/143） and disease control rate of 93.71% （134/143） compared to the control group， with an objective response rate of 55.94% （80/143） and disease control rate of 83.92% （120/143） （P < 0.05）. After treatment， both groups showed significant reductions in TCM symptom scores and serum levels of CYFRA21－1， CEA， and significant increases in IL－6， TNF－α， and TGF－β1 levels （P < 0.05）. The observation group had significantly lower TCM symptom scores and lower levels of CYFRA21－1， CEA， IL－6， TNF－α， and TGF－β1 compared to the control group （P < 0.05）. The adverse reaction rate in the observation group was 14.69% （21/143）， significantly lower than the control group’s 28.67% （41/143） （P < 0.05）.Conclusion： Yangyin Qingfei Huoxue Formula has significant clinical effects in treating radiation－induced lung injury in NSCLC patients with Qi－Yin deficiency syndrome after radiotherapy. It improves TCM clinical symptoms， reduces inflammatory factors and tumor marker levels， and alleviates adverse reactions. |
