      目的:评价接骨柔筋方的药物安全性并探讨其促进胫骨骨折模型大鼠成血管、改善骨代谢的作用机制。方法:SD大鼠制备胫骨骨折模型,造模成功后随机分为模型组、接骨柔筋方组及仙灵骨葆组,每组15只,另取15只大鼠设为空白组。空白组不进行任何干预措施;模型组给予生理盐水10 mg/(kg·d)灌胃;仙灵骨葆组按照10 mg/(kg·d)灌胃仙灵骨葆混悬液;接骨柔筋方组按照13.5 mg/(kg·d)灌胃,均每日2次,干预2周。采用生化检测血清肌酐、尿素氮评价药物安全性;采用生化检测碱性磷酸酶、钙、磷、睾酮水平评价接骨柔筋方对骨代谢的影响;采用血液流变学检测纤维蛋白原、红细胞沉降率、血浆黏度和红细胞压积评价接骨柔筋方干预后血液流变学改变;采用CD31免疫荧光染色检测骨折断端软组织微血管密度评价其成血管作用。结果:与模型组比较,仙灵骨葆组和接骨柔筋方组大鼠全血中肌酐、尿素氮水平降低(P < 0.05),碱性磷酸酶、钙、磷、水平升高(P < 0.05),红细胞沉降率、血浆黏度和红细胞压积显著降低(P < 0.05),骨折断端腓肠肌及比目鱼肌CD31阳性微血管比例增加(P < 0.05),接骨柔筋方组睾酮水平和纤维蛋白原含量显著降低(P < 0.05),仙灵骨葆组睾酮水平和纤维蛋白原含量有下降趋势,但差异无统计学意义(P > 0.05)。结论:接骨柔筋方药物安全性良好,具有促进成血管、改善血液流变学、改善骨代谢等作用,其促进骨折愈合的作用机制可能与其成血管以及改善骨代谢有关。
英文摘要:
      Objective: To evaluate the drug safety of Jiegu Roujin Formula and explore its mechanism in promoting angiogenesis and improving bone metabolism in rats with tibial fracture. Methods: SD rats were used to establish tibial fracture models. After successful modeling, rats were randomly divided into model group, Jiegu Roujin Formula group, and Xianling Gubao group (15 rats each), with an additional 15 rats as blank group. The blank group received no intervention; the model group received saline [10 mg/(kg·d)] by gavage; the Xianling Gubao group received Xianling Gubao suspension [10 mg/(kg·d)]by gavage; and the Jiegu Roujin Formula group received the formula [13.5 mg/(kg·d)]by gavage, all administered twice daily for 2 weeks. Drug safety was assessed by serum creatinine and urea nitrogen levels; Bone metabolism was evaluated by alkaline phosphatase, calcium, phosphorus, and testosterone levels; Hemorheological changes were measured by fibrinogen, erythrocyte sedimentation rate, plasma viscosity, and hematocrit after the Jiegu Roujin Formula intervention; Angiogenesis was assessed by CD31 immunofluorescence staining of microvessel density in fracture site soft tissues. Results: Compared with the model group, both Xianling Gubao and Jiegu Roujin Formula groups showed decreased creatinine and urea nitrogen levels (P < 0.05), increased alkaline phosphatase, calcium, and phosphorus levels (P < 0.05), significantly reduced erythrocyte sedimentation rate, plasma viscosity, and hematocrit (P < 0.05), and increased CD31-positive microvessel proportion in gastrocnemius and soleus muscles at the fracture site (P < 0.05). The Jiegu Roujin Formula group additionally demonstrated significantly lower testosterone and fibrinogen levels (P < 0.05),although the there are descending trend for testosterone level and fibrinogen in Xianling Gubao group, the difference was not statistically significant(P > 0.05). Conclusion: Jiegu Roujin Formula demonstrates good safety and effects in promoting angiogenesis, improving hemorheology, and enhancing bone metabolism. Its fracture-healing mechanism may involve angiogenesis promotion and bone metabolism regulation.
