| 目的：探讨通督温阳针法联合放散式冲击波对寒湿痹阻型腰椎间盘突出症（LDH）的疗效，并观察其对C反应蛋白、前列腺素E2、白细胞介素－6和5－羟色胺水平的影响。方法：选择河南平煤神马医疗集团总医院康复医学科符合纳入标准的寒湿痹阻型LDH患者90例，依据随机数字表随机分为联合治疗组和对照组，每组各45例。对照组给予放散式冲击波治疗，联合治疗组给予通督温阳针法联合放散式冲击波治疗，两组患者均治疗1个月。观察治疗前后两组患者Oswestry功能障碍指数（ODI）、前列腺素E2（PGE2）、VAS评分、白细胞介素－6（IL－6）、血清C反应蛋白（CRP）、5－羟色胺（5－HT）的变化情况，并比较临床疗效和复发率。结果：对照组临床总有效率为75.56%（34/45），联合治疗组临床总有效率为93.33%（42/45），差异具有统计学意义（P < 0.05）。治疗后两组患者ODI 指数、VAS 评分和CRP、IL－6、PGE2、5－HT水平均降低，且联合治疗组上述指标改善程度优于对照组（P < 0.05）。联合治疗组复发率为0%（0/42），对照组复发率为26.47%（9/34），差异具有统计学意义（P < 0.05）。结论：通督温阳针法联合放散式冲击波可有效缓解寒湿痹阻型LDH患者腰腿疼痛的临床症状，降低机体炎症因子和疼痛介质表达，改善腰椎功能。 |

| Objective： To investigate the efficacy of Tongdu Wenyang acupuncture combined with radial shock wave therapy for lumbar disc herniation （LDH） with cold－dampness obstruction pattern， and to observe its effects on levels of C－reactive protein， prostaglandin E2， interleukin－6， and serotonin. Methods： Ninety patients with LDH of cold－dampness obstruction pattern meeting the inclusion criteria in the Department of Rehabilitation Medicine of Henan Pingmei Shenma Medical Group General Hospital were selected and randomly divided into a combination treatment group and a control group according to a random number table， with 45 cases in each group. The control group received radial shock wave therapy， while the combination treatment group received Tongdu Wenyang acupuncture combined with radial shock wave therapy. Both groups were treated for 1 month. Changes in Oswestry Disability Index （ODI）， prostaglandin E2 （PGE2）， VAS score， interleukin－6 （IL－6）， serum C－reactive protein （CRP）， and serotonin （5－HT） were observed before and after treatment in both groups， and clinical efficacy and recurrence rates were compared. Results： The total clinical effective rate was 75.56% （34/45） in the control group and 93.33% （42/45） in the combination treatment group， with a statistically significant difference （P < 0.05）. After treatment， ODI index， VAS scores， and levels of CRP， IL－6， PGE2， and 5－HT decreased in both groups， and the improvement in these indicators in the combination treatment group was greater than that in the control group （P < 0.05）. The recurrence rate was 0% （0/42） in the combination treatment group and 26.47% （9/34） in the control group， with a statistically significant difference （P < 0.05）. Conclusion： Tongdu Wenyang acupuncture combined with radial shock wave therapy can effectively alleviate clinical symptoms of back and leg pain in patients with LDH of cold－dampness obstruction pattern， reduce the expression of inflammatory factors and pain mediators， and improve lumbar function. |
