      目的 观察比较指按法、针刺及CHEPs刺激器作用于正常人手三里穴产生的痛温觉诱发电位(CHEPs),探析经络理论的小纤维神经感传方式。 方法 本文采用自身前后对照设计,纳入的16名健康成人,收集按法、针刺、CHEPS刺激器作用于手三里穴时产生的痛觉诱发电位。 结果 3组刺激手段作用于16名志愿者手三里穴均可记录到痛温觉诱发电位,按法组诱发的N波潜伏时比CHEPS组的显著高120.69ms(P<0.05),针刺组诱发的N波潜伏时比CHEPS组的显著高183.69ms(P<0.001)。按法组诱发的P波潜伏时比CHEPS组的显著高149.13ms(P<0.05),针刺组诱发的P波潜伏时比CHEPS组的显著高215.69ms(P<0.001),均为未发现按法组和针刺组N、P波潜伏时具有统计学差异。未见不同干预手段对N、P波幅存在显著主效应(F=0.424,P=0.658)F=1.289,P=0.29)。 结论(1)按法、针刺均可诱发由小纤维介导的N、P波,且通过与CHEPS诱发电位相比较,按法及针刺传导的小纤维速度更慢,为无髓鞘的C纤维;(2)按法、针刺诱发的N、P波波幅与CHEPS组相比,不影响痛觉诱发电位的波幅。

      Objective To observe and compare the pain and heat evoked potentials produced by Acupuncture and CHEPs stimulator acting on Sanli point of normal hand, and to explore the small fiber nerve conduction effect of meridian theory. Method In this study, 16 healthy adults were enrolled in a self-controlled design. Contact Heat evoked potentials (CHEPs) were collected when acupuncture, acupuncture and CHEPS stimulator acted on Three Miles of Hand. Result Pain-temperature evoked potentials were recorded in all 16 volunteers treated with three stimulation methods in Handsanli Point. The latency time of N-wave evoked by the method group was 120.69ms higher than that of the CHEPS group(P < 0.05). The N wave latency induced by acupuncture group was 183.69ms higher than that of CHEPS group (P < 0.001). The P-wave latency induced by acupuncture group was significantly higher than that induced by CHEPS group from 149.13 MS (P < 0.05). The P-wave latency caused by acupuncture group was 215.69ms higher than that induced by CHEPS group (P < 0.001). Learning differences. There was no significant main effect (F=0.424, P=0.658) F=1.289, P=0.29) in N and P amplitude of different intervention methods. Conclusion (1) Finger Pressing and Acupuncture both can induce N and P waves mediated by small fibers, and compared with CHEPPS evoked potentials, the velocity of small fibers transmitted is slower, and they are unmyelinated C fibers; (2) There is no significant difference in the amplitude of N and P waves induced by Finger Pressing and Acupuncture compared with CHEPS group.
