      血管性痴呆是继阿尔茨海默病后的第2大痴呆性疾病,是迄今为止唯一可以防治的痴呆。慢性脑低灌注是血管性痴呆的一个重要原因,血管性痴呆的病理机制迄今为止仍未完全阐明,目前FDA仍无批准用于治疗血管性痴呆的药物。线粒体氧化应激与脑缺血后神经细胞凋亡坏死密切相关,神经细胞缺血缺氧时氧化应激诱导线粒体凋亡途径,在 VD 发生发展过程中具有重要作用。中药具有多靶点、多途径治疗特征,在 VD 防治方面具有独特优势,本文通过系统复习国内外文献,发现目前中医药基于线粒体保护方面防治血管性痴呆的机制包括减轻神经元线粒体水肿、增加线粒体数量、提高ATP酶活力、提高线粒体呼吸链关键酶的活力、通过线粒体凋亡通路启动抗凋亡机制、提高线粒体复合物的活性等方面。目前已经有一定数量的研究探索中医药基于线粒体保护防治VD的机制,将来可基于线粒体稳态,进行更深入的研究,如研究基于现代中药新药创制理论的中药组分对线粒体的调节作用;基于线粒体稳态的新近重要信号通路进行研究,以便筛选出针对线粒体稳态的疗效卓著的中药组分或单体,助推治疗血管性痴呆的新药创制。

      Vascular dementia, the second largest dementia disease after Alzheimer"s disease, is the only preventable dementia so far. Chronic cerebral hypoperfusion is an important cause of vascular dementia. The pathological mechanism of vascular dementia has not been fully elucidated up to now. At present, the FDA has not approved drugs for the treatment of vascular dementia. Mitochondrial oxidative stress is closely related to neuronal apoptosis and necrosis after cerebral ischemia. The oxidative stress induced mitochondrial apoptosis pathway plays an important role in the occurrence and development of VD. Traditional Chinese medicine (TCM) has the characteristics of multi-target and multi-pathway, and has unique advantages in VD prevention and treatment. This paper systematically reviewed the literature at home and abroad and found that the mechanisms of TCM in preventing and treating vascular dementia based on mitochondrial protection include reducing neuronal mitochondrial edema, increasing the number of mitochondria, increasing ATPase activity, increasing the activity of key enzymes in mitochondrial respiratory chain, anti-apoptotic through anti-mitochondrial apoptotic pathway,and enhances the activity of mitochondrial complex. At present, a number of studies have explored the mechanism of mitochondrial protection and prevention of VD in traditional Chinese medicine. In the future, more in-depth studies based on mitochondrial homeostasis can be carried out, such as the protection effect on mitochondria of traditional Chinese medicine based on the new drug creation of modern Chinese medicine such as component compatibility theory; and the recent important signaling pathways based on mitochondrial homeostasis, so as to screen out effective components or monomers of traditional Chinese medicine which would promote the development of new drugs for treating vascular dementia.
