随着中医药事业的发展与进步,中医药在防治骨质疏松症中发挥着重要作用,传统中医理论多以“虚证”立论,然而“虚证”与“瘀证”联系紧密,在骨质疏松症的辨证中应当对“瘀证”同样重视。细胞铁死亡是一种铁依赖性的,脂质过氧化反应物堆积的细胞死亡方式,被学界认为是现代细胞生物学的一大突破。谷胱甘肽过氧化物酶4(glutathione peroxidase 4,GPX4)是当前公认的调节细胞铁死亡的通路之一,GPX4通路可抑制人体细胞铁死亡中关键物质细胞内活性氧堆积,并对细胞铁死亡引起的组织特异性消融起重要调控作用,是目前研究细胞铁死亡的主要方向。国内外学者针对此通路做了大量研究,笔者查阅文献对GPX4通路介导破骨细胞、成骨细胞铁死亡在骨质疏松症中的发病机制,中医“瘀证”理论与GPX4通路调控细胞铁死亡和骨质疏松症之间关系进行总结,发现“瘀证”理论指导下运用活血化瘀类方药可以干预GPX4通路调控细胞铁死亡,对“骨代谢稳态”产生影响,在骨质疏松症防治及后续研究中具有重大意义。 |

With the development and progress of traditional Chinese medicine, traditional Chinese medicine plays an important role in the prevention and treatment of osteoporosis. Traditional Chinese medicine theory is mostly based on ' deficiency syndrome '. However, ' deficiency syndrome ' is closely related to ' stasis syndrome ', and ' stasis syndrome ' should be paid equal attention to in the syndrome differentiation of osteoporosis. Cell ferroptosis is an iron-dependent, lipid peroxidation reactant accumulation of cell death, is considered a major breakthrough in modern cell biology. Glutathione peroxidase 4 ( GPX4 ) is currently recognized as one of the pathways regulating cell ferroptosis. The GPX4 pathway can inhibit the accumulation of reactive oxygen species in the key substances of human cell ferroptosis, and plays an important regulatory role in tissue-specific ablation caused by cell ferroptosis, which is the main direction of cell ferroptosis research. Scholars at home and abroad have done a lot of research on this pathway. The author reviewed the literature on the pathogenesis of GPX4 pathway-mediated ferroptosis of osteoclasts and osteoblasts in osteoporosis, and summarized the relationship between the theory of ' stasis syndrome ' in traditional Chinese medicine and the regulation of GPX4 pathway on ferroptosis and osteoporosis. It was found that the use of blood-activating and stasis-dissolving prescriptions under the guidance of the theory of ' stasis syndrome ' could interfere with the regulation of GPX4 pathway on ferroptosis and affect the ' bone metabolism homeostasis ', which was of great significance in the prevention and treatment of osteoporosis and subsequent research. |
