| 目的:系统性评价健脾养正消癥方配合紫杉醇类化疗对进展期胃癌预后生存的影响。方法:选取2011年2月—2014年2月江苏省中医院和5个分中心收治的符合纳入标准胃癌患者72例,随机分为观察组32例和对照组40例。对照组单纯使用紫杉醇类化疗药物,观察组在对照组治疗基础上联合健脾养正消癥方进行治疗。以患者死亡定义为本次事件终点,观察患者总生存期、死亡风险和中医证候疗效。结果:观察组患者总生存期为(413.88±21.68)d,对照组患者总生存期为(332.00±22.90)d,两组比较差异具有统计学意义(P=0.03 <0.05)。经Cox回归分析得到,观察组较对照组的死亡风险明显下降(P <0.05)。观察组治疗2个月和4个月的中医症候积分低于对照组(P <0.05,P <0.01),且观察组中医证候疗效(56.3%, 18/32)显著优于对照组(25.0%, 10/40)(P <0.05)。结论:健脾养正消癥方联合紫杉醇类化疗可以有效延长进展期胃癌患者的总生存期,改善患者的临床症状,提高进展期胃癌患者的生活质量。 |

| Objective: To evaluate the effect of Jianpi Yangzheng Xiaozheng Decoction combined with paclitaxel chemotherapy on the prognosis and survival of advanced gastric cancer. Methods: A total of 72 patients with gastric cancer were treated in Jiangsu province hospital of Chinese medicine and five sub-centers from 2011 to 2014. Patients were divided into two groups, including experiment group (32 cases) and control group (40 cases). The experiment group was treated with Jianpi Yangzheng Xiaozheng Decoction combined with chemotherapy, while the control group was treated with chemotherapy alone. The changes of overall survival (OS), death risk and clinical symptom scores were observed. Results: The average survival period of the experiment group was (413.88±21.68) days and that of the control group was (332.00±22.90) days (P=0.03). The death risk of patients in the experiment group decreased significantly, compared with those in the control group (P＜0.05). Through the comparison of TCM symptom scores, the TCM efficacy of the experimental group was significantly better than that of the control group (P＜0.05). Conclusion: Jianpi Yangzheng Xiaozheng Decoction combined with paclitaxel chemotherapy could prolong the overall survival of patients with advanced gastric cancer, improve the clinical symptoms and significantly make the quality of life better of patients. |
