| 目的：观察温心炙草汤对乳腺癌阿霉素化疗所致心脏毒性的影响。方法：将68例接受4周期阿霉素辅助化疗的早期浸润性乳腺癌心阳虚证患者依据随机数字表随机分为温心组(34例)和对照组(34)例，温心组脱落1例，对照组脱落2例，最终完成方案温心组33例，对照组32例。温心组从第2周期开始每周期化疗后口服温心炙草汤2周，对照组给予对应模拟剂，其他治疗两组相同。观察化疗前后两组患者KPS、MDASI-TCM评分和心脏毒副反应程度、心肌及心衰标志物(肌钙蛋白、肌酸激酶同工酶水平、前端脑钠肽及心电图改变率的变化情况。结果：两组化疗前KPS、MDASITCM、cTnI、CK-MB比较差异均无统计学意义(P>0.05)；对照组化疗后KPS评分低于化疗前(P<0.05),MDASI-TCM评分、cTnI、CK-MB高于化疗前(P<0.05)；温心组化疗后MDASI-TCM评分、CK-MB高于化疗前(P<0.05)；且化疗后，温心组KPS评分高于对照组(P<0.05),MDASITCM评分、cTnI、CK-MB低于对照组(P<0.05)。温心组心脏毒性发生率、NT-BNP和心电图改变率低于对照组(P<0.05)。结论：温心炙草汤可降低乳腺癌阿霉素化疗所致心脏毒性，改善心脏功能。 |

| Objective:To observe the effect of Wenxin Zhicao Decoction(WZD) on cardiac toxicity induced by adriamycin chemotherapy for breast cancer.Method:68 cases of early invasive breast cancer patients with heart yang deficiency syndrome who received four courses of adriamycin adjuvant chemotherapy were randomly divided into WZD group(n = 34) and control group(n = 34) according to random number table.1 patient in WZD group and 2 in control group dropped out,and 33 patients in WZD and 32 in control group completed the study.Starting from the second course,WZD group received oral WZD for 2 weeks after each course of chemotherapy,while the control group received corresponding simulated agents,and other treatments were the same in both groups.Before and after the chemotherapy,KPS and MDASI-TCM scores,degree of cardiac toxicity,myocardial and heart failure markers(troponin,creatine kinase isoenzyme,B-type natriuretic peptide),and electrocardiogram changes were observed in both groups.Results:There was no statistically significant difference in KPS,MDASI-TCM,cTnI and CK-MB between the two groups before chemotherapy(P>0.05).After treatment,KPS score decreased(P>0.05),and MDASI-TCM score,cTnI and CKMB increased(P>0.05) in control group.MDASI-TCM score and CK-MB in the WZD group after chemotherapy were higher than those before chemotherapy(P<0.05) 05).MDASI-TCM score and CKMB in WZD group after chemotherapy were higher than those before chemotherapy(P<0.05).Furthermore,after chemotherapy,KPS score of WZD group was higher than that of the control group(P<0 05),MDASITCM score,cTnI,and CK-MB were lower than those in control group(P<0 05).The incidence of cardiotoxicity,NP-BNP,and electrocardiogram changes in WZD group were lower than those in control group(P<0 05).Conclusion:Wenxin Zhicao Decoction can reduce the cardiac toxicity caused by adriamycin chemotherapy in breast cancer and improve cardiac function. |
