| 糖尿病微血管病变具有隐匿发病过程与严重不良结局的致病特征，危害严重。固本培元学说源远流长，是众多新安医家在临床实践中探索出的以“调补人体元气、顾护脾肾之本”为主要治法的思想理论。本文对糖尿病微血管并发症的中医认识和固本培元的理论渊源进行了介绍，并对新安医家治疗糖尿病微血管并发症的相关文献进行总结分析，从脾肾论治角度入手，探讨了糖尿病微血管病变与脾、肾之间的关系，并阐述了固本培元思想在糖尿病微血管并发症防治中的作用。笔者认为脾肾亏虚致血运无源，血行无力是导致瘀血形成、阻滞脉行从而引起糖尿病微血管病变发生的主要原因。众多新安医家在“重视培补先后天之本、脾肾同治”理论指导下中药复方的配伍使用在糖尿病微血管病变中能够发挥多途径、多靶点、整体调控的治疗优势，对中医药临床防治糖尿病及其微血管并发症有重要意义。 |

| Microangiopathy in diabetes has the pathogenic characteristics of hidden pathogenesis and serious adverse outcomes, which is a serious hazard. The theory of consolidating root and cultivating source(CRCS) has a long history, and is an ideological theory explored by many Xin'an medical practitioners in clinical practice. Its main treatment method is "regulating and nourishing body source qi and consolidating the root of the spleen and kidney". This article introduces the TCM understanding of diabetes microvascular complications and the theoretical origin of CRCS, and summarizes and analyzes the relevant literature of Xin'an physicians on the treatment of diabetes microvascular complications. From the perspective of treatment of spleen and kidney, we discussed the relationship between the microvascular disease of diabetes and the spleen and kidney, and expounded the role of the thought of CRCS in the prevention and treatment of microvascular complications of diabetes. We believe that the deficiency of spleen and kidney leads to shortage of blood source and weakness of blood circulation, resulting in blood stasis which inhibits blood circulation, giving rise to the occurrence of microvascular disease of diabetes. Many Xin'an physicians, under the guidance of the theory of "attaching importance to the cultivation and tonification of the root of innate endowment and the foundation of acquired constitution, and simultaneous treatment of the spleen and kidney", use the combination of Chinese medicinals in the treatment of diabetes microvascular disease to give play to the therapeutic advantages of multiple ways, multiple targets and overall regulation, which is of great significance to the clinical prevention and treatment of diabetes and its microvascular complications. |
